Please Note: Our Civil Surgeon will not sign off on any immigration physicals unless the applicant (patient) has had their covid vaccines. We do not sign exceptions. Thank you so very much.
You are recommended to sign-in online at HilltopMedicalClinic.com from 7am-11am, 7 days per week, and we will perform your physical between 10am-3pm that day. Again, sign-in the day you would like to do your physical. Please be sure to bring vaccine records, official photo ID and if you were married and your name has changed, bring your official marriage certificate as well.
The cost ranges from $450- $600 depending on vaccines & lab needing performed. Please be sure to allow 7-10 business days (excludes weekends) for the full completion of your forms. We will call you to return to sign the original form and that will be placed in a sealed envelope. You will also be given a “copy” of your physical so you will know exactly what is in your sealed envelope.
